Sonoma County moved up two places in the overall ranking for Health Outcomes. In 2016, the county is ranked sixth, according to the University of Wisconsin Population Health Institute and Robert Wood Johnson Foundation’s annual County Health Rankings and Roadmaps Report.

This isn’t by accident. Health Action, a collaboration of nonprofits and government partners, led by the Department of Health Services, has been actively focusing on broad factors that influence health, with particular attention to addressing health disparities on a local level. These critical efforts are marking improvements in health and well-being for all Sonoma County residents.
